Summary points:

  1. Forty-four percent of decedent patients enrolled in a home ventilation service had completed an advance care plan (ACP) in their lifetime.
  2. Among decedent home ventilation patients who had completed an ACP, 44.2% of these plans were completed more than 1 year prior-to-death, and 78.9% were completed after enrollment with a homecare service.
  3. Decedent home ventilation patients with an ACP consumed <55% in cumulative length of stay in an acute hospital (AH) in the final 90 days of life, compared to those without an ACP.
